Before you submit this form to have fluorescent tubes removed from your site, please make sure of the following:

1. Your lamps are properly contained in a secure barrel or box. 
   
2. You have a MINIMUM of 200 bad fluorescent tubes.

When picked up, each barrel or box of tubes will be reaplced with an empty, secure container.
